Immunization Requirements for the 2019-20 School Year
K.A.R. 28-1-20 defines immunizations required for any individual who attends a school or childcare program operated by a school. Following is a list of the immunization requirements for the 2019-20 school year. The usual number of doses required are listed; however, there are exceptional circumstances that could alter the number of doses a child needs. If you have questions about your child’s immunization status, contact your health care provider or the local health department. Written documentation of immunization status must be provided to the school prior to the first day of school.

Varicella(chicken pox) 1 dose*
Hepatitis A 2 doses
Hepatitis B 3 doses
Hib(haemophilus influenza type B) 4 doses
Prevnar(pnuemococcal conjugate) 4 doses
K-Grade 1 DTaP/DT 5 doses
IPV 4 doses
MMR 2 doses
Varicella 2 doses*
Hepatitis A 2 doses
Hepatitis B 3 doses
Grades 2 - 6 DTaP/DT 5 doses
IPV 4 doses
MMR 2 doses

* Varicella (chicken pox) vaccine is not required if the child has had the chicken pox
disease and the disease has been documented with a physician’s signature. Without a
physician’s signature, the vaccine is required.
